Information for receiving donations for your registered scfvd from the foundation for veterinary dentistry

Requirements for Execution and Receipt of Benefits:

To receive the benefits, the following is required of the student chapter:

New Chapters

Newly established SCFVD chapters must fill out the new chapter form with club information, and the information for receiving donation funds form (who the check needs to be addressed to and mailing address).

Existing Chapters

Must fill out their annual report by November 30th.
